How to Register


  • If you wish to register with our practice you should approach the receptionists who will guide you through the process.
  • The Primary Care Trust advise that practices should define a "practice boundary" for patient acceptances. Our practice area covers the entire length of the Whitworth Valley from the end of New Line in Stacksteads to Howard Street in Rochdale. The Practice Management team can advised on the status of addresses on the periphery of our area for registration.
  • You will be required to either complete a GP registration form , or complete and sign your NHS medical card and leave it for the administration team to process.
  • Adults will be asked to complete a new patient questionnaire which details significant medical history including allergies and medications. If you are on medications or have a complex medical history , a new patient check should be arranged with one of the doctors so that repeat prescriptions can be established and disease monitoring set up.
  • Your registration form will ask for details of your previous practice. When this is processed your old practice will forward all of your GP records to Whitworth Medical Centre. For paper records this often taken several weeks as the records pass through the Primary Care Trust(s) before transfer. Some practices now are able to forward electronic records much more quickly.
